Before Your First Visit

1. Book and confirm your first appointment.

2. Open Welcome e-mail from JaneApp and create an account.

3. Next, fill out all patient intake forms from JaneApp that were e-mailed to you.

4. Upload any x-rays, MRIs, lab tests, or dental films relevant to your issues to your JaneApp account or email to: info@cliffmatsuno.com

5. Please bring or wear loose clothing, sports bra and either shorts or sweats on the day of your visit.

6. Eat light before and heavier after visit.

7. Please have payment readily available.

8. Arrive 15 minutes early on your first visit.
